Intake Coordinator — Care Partner Institute (CPI) / Care Connection Resource Center (CCRC)
 
Nevada Senior Services (NSS) is seeking an individual to join our team as an Intake Coordinator. As a member of the CPI/CCRC team, this individual performs a variety of duties and responsibilities related to consumer intake, database fidelity and input, administrative support, data collection and reporting. The Intake Coordinator is an experienced professional responsible for supporting process and programmatic needs of the CPI/CCRC.
 
  • Assist all programs and CPI/ CCRC staff to provide services that engage consumer and provide quality assistance to meet the needs of each individual being served.
  • Completes all consumer intake, screening and triage duties and connects consumer need to service program.
  • Ability to evaluate the need for community services and make appropriate referrals, based on organizational protocols.
  • Provides administrative support to all CPI/CCRC programs and services.
 
Professional Requirements:
Minimum Qualifications: Social Service professional with associate’s degree from an accredited college; related and equivalent experience accepted. Social service work with chronic disease, disability and/or aging population and working knowledge of community resources/benefits preferred.

Professional Social Worker/Care Consultant
 
Innovative, non-profit organization in Las Vegas, NV has a full time position available for a professional Social Worker/Care Consultant. Nevada Senior Services is dedicated to providing programs, services and research initiatives aimed at improving the physical, spiritual, and emotional health of individuals and their care partners who are facing the challenges of chronic disease, disability and aging.
 
The Care Consultant performs a variety of duties and responsibilities related to an evidence-based telephone coaching intervention. As a member of the Care Partner Institute /Care Connection Resource Center Team this individual provides caregiver support including consultation, assessment, information, referrals and support services to individuals with chronic health conditions and/or their care partners. Desired professional, empowers and advocates for such individuals and/or their care partners to ensure identified needs reflect abilities/ strengths.
Minimum Qualifications: Bachelor’s Degree in health or social services from an accredited college with one-year practical experience in the field desired; work related to chronic disease, disability and/or aging population preferred.

JOB TITLE: Respite Coach
 
JOB SUMMARY: Providing short-term, in-home, non-medical Respite services to care partners who care for individuals who have dementia and have been recently hospitalized.  Short-term intensive respite service provides care partners a break from caregiving responsibilities immediately post hospitalization.

Part time RN (20-25 hours) with passion for geriatrics and disability. Joyful state of the art program setting that dares to care!

The Adult Day Care Center of Las Vegas serves persons over 18 years of age who have medical, physical, social and/or cognitive disabilities. The Center is located in Las Vegas at Washington and Jones. Hours of operation are 6:30am -- 6:30pm M-F and 8:00am -- 6:00pm Saturday and Sunday excluding major holidays.

Basic Requirements:
Current Nevada RN Nursing License
Current CPR Certification

Pre-employment process, requires all employees to have state mandated fingerprint background checks, current 2-step TB test, and a physical within the last 6 months.
