Care Transition Specialist—Care Partner Institute (CPI) / Care Connection Resource Center (CCRC)

Nevada Senior Services (NSS) Care Partner Institute is seeking an individual to join our team as a Care Transitions Specialist. As a member of the CPI/CCRC team, this individual performs a variety of duties and responsibilities aiming to reduce hospital readmissions, improving health outcomes and access to services. Care Transitions are generally defined as the movement of an individual from one care setting (such as a hospital, long-term care setting or rehabilitation facility) to another, often home.

The Care Transition Specialist will provide a comprehensive person-centered, social work-led, interdisciplinary transitional care intervention that addresses issues resulting from hospitalization. Care Transitions Specialist will seek to provide better care outcomes by assisting individuals and their families during transitions of care.

The Care Transition Specialist is an experienced professional with a background in providing services within healthcare and/or community based settings, serving individuals living with Alzheimer’s disease and related dementias; and their families.

Professional Requirements:
Minimum Qualifications:
Master’s degree in Social Work from an accredited college, 2-5 years practical experience in healthcare, social services or a related field desired; Equivalent combination of education and experience accepted. Work related to Alzheimer’s disease and related dementias, chronic disease, disability and/or aging population and caregivers, preferred.

Part time RN (20-25 hours)

Part time RN (20-25 hours) with passion for geriatrics and disability. Joyful state of the art program setting that dares to care!

The Adult Day Care Center of Las Vegas serves persons over 18 years of age who have medical, physical, social and/or cognitive disabilities. The Center is located in Las Vegas at Washington and Jones. Hours of operation are 6:30am -- 6:30pm M-F and 8:00am -- 6:00pm Saturday and Sunday excluding major holidays.

Basic Requirements:
Current Nevada RN Nursing License
Current CPR Certification

Pre-employment process, requires all employees to have state mandated fingerprint background checks, current 2-step TB test, and a physical within the last 6 months.
